What happens after The Commons?
The Commons at WestBridge is located in Manchester, NH. WestBridge offices and community-based care teams are located in Manchester, NH and Medford, MA; however WestBridge provides clinical services throughout the region. In addition, WestBridge provides Consultation services anywhere in the United States.
Do you take participants from anywhere in the country?
Yes. We accept referrals from anywhere in the United States. If appropriate for our services, we are willing and able to travel to your home to further assess for and coordinate admission to, our programs.
What is the length of stay?
Participants at The Commons typically stay anywhere from one to six months, but this is determined on an individual basis, as not all our participants needs are the same. Some stays may be shorter, some may be longer.
How soon can an admission happen?
WestBridge's goal is to be as thoughtful as possible when admitting a new participant. We strive to be responsive and efficient as well. We want the admission process to provide the interested participant with all of the information about our program, as well as gather all the appropriate information from the participant so that we can determine if WestBridge is the best fit.
What happens after The Commons?
As the participant's stay nears the end, the participant works with their team to devise a Transition Plan. Sometimes Transition Plans involve returning to their home town or state, sometimes participants choose to remain in the Manchester, NH, or Boston area, and continue with our community-based care teams. Either way, WestBridge assists the participant and family with making the transition a safe, smooth and productive one.
